All specialist referral requests must be submitted through the Referral Tracking system:

In order to submit a referral request or check on patient status, you must be a registered user with a pre-authorized username and password.

When logging in for the first time, you will be asked to change the default password to your own private password.

If you have trouble logging on, please call IT Help Desk at 437-4503 (x44503) to have your password reset or to have yourself enabled again.

For Non-Web Based Health Professionals

  1. Please visit Referral Tracking System to download the referral request form. 
  2. Visit the “Guidelines” tab to make sure referral request include all pre-requisite materials, tests, etc. Failure to comply by the Guidelines will result in delay of patient being seeing by the specialist.
  3. Fax the completed form to 510.437.5180 to Referral Unit.

If you would like to register for the Referral Tracking System, please download and complete the non-AHS employee forms from the homepage at and then submit your request to LaTisha Murray, Director of Ambulatory Call Center and Referral Unit
